Konza Prarie Community Health Center Dental Screenings

To parents/guardians,

 Konza Prarie Community Health Center will again be working with us on providing dental screening services at our school on October 6th.  There is NO out of pocket cost to you for the services.  If you just want the screening performed you do not need to return the form, if we do not receive the form we are under the assumption that you want your child having the screening performed.   If you want the fluoride treatment performed we DO NEED the form filled out and returned.  If you absolutely DO NOT want this screening service at all please write NO on letter and sign it and send it back with your child.  The procedure takes about 30 seconds as the dental Hygenist(s) look at your child’s teeth/gums.  I will send an email/letter home regarding the findings of the screenings.  Please have these forms returned to me by September 30th.  Have your child give them to their teacher, or take them to the front office.  Again if there are any questions, or concerns, please feel free to reach to me at my email jtegtmeier@usd223.org Jeri Tegtmeier, RN, (USD 223 school nurse)

We are working with the Washington County Health Department and we are planning a vaccine clinic on 10/14 for the Linn students and staff.  We will be offering Influenza and COVID vaccines.  If you choose to have your child/ren, self receive a vaccine, there will be a link from the Washington County Health Department that you may fill out and sign.  Please do this by October the 10th of October so we know how many vaccines we will need.  There will be a consent form for each child/person so please fill them out appropriately.
