Konza Prairie Community Health Center is coming to both Linn and Hanover next Wednesday, November 11, to provide free dental screenings. You may opt out of the screenings if you prefer, and you may have already done so at time of registration. Information regarding screening is linked below.
They also offer fluoride application. For your child(ren) to receive this service, please print off, complete, and return the consent form in English or Spanish linked below, giving your consent.
