We are holding a Vaccine clinic at the Hanover schools on Friday October the 7th for students. Below you will find a link that will need to be filled out by October 3rd for each child. This link is for the Influenza vaccine. We will be providing the Influenza vaccine and the COVID booster at the clinic. If you choose to have a COVID booster, you will need to call the Washington County Health Department and they will send you the link for the COVID booster consent form. Again if you choose to have your child/ren receive any of these vaccines, each child will need the consent form filled out. IF you can send their Immunization card with them we can get them updated.
