Konza Prairie Community Health Center is performing dental screenings for grades Pre-K - 6th on November 19th. The screening is very quick and just involves a visual inspection. Forms did come home with students for parents to fill out if they would like their child to receive fluoride. The forms need to be returned by November 15th. If you want your child to receive fluoride, fill out and return the form. If you do not want fluoride and just want your child screened, you don’t have to do anything. If you do NOT want your child screened, you need to let the school know! Write a little note on the form and send it back is the easiest.
